Intake forms:
Please click on the "Intake form" icon to download an intake sheet to bring with you to your first session. This will allow for more counseling time and less paperwork upon first appointment. Alternatively, you can email forms to the administrative assistant and if you would like to inquire about insurance coverage, you may email Kylie Smith and send her a copy, front and back, of your insurance card. Carla is an out-of- network provider which means she is not paneled with insurance companies. Many insurance companies, however, will reimburse the client for fees at a lower percentage than if Carla were an in-network provider. As a courtesy to our clients, we will file insurance claims electronically for client reimbursement. All fees are paid at the time of service, and your understanding of a 24 hour cancellation policy is appreciated and will be enforced.
